HealthMap Vaccine Finder

This online tool is an aid to identifying which vaccines a person in the United States (US) might need and where to find them. It is an interactive site that includes a map, a list of vaccines (see below), and a locator that gives an address of available vaccines of the type checked by the user in the list.
- Flu:
Trivalent Flu Shot

As noted on the site, a site user "can also contact your physician for vaccination. Locations can run out of a vaccine. Please call ahead to confirm the vaccine is in stock." In order to help users determine what vaccines they might need, a quiz asks for age, gender, and location and then offers to help identify individual vaccine needs or whether a flu vaccine is appropriate, through questions such as:
"4. Are you currently pregnant?
yes no
5. Are you immunocompromised due to chemotherapy or a non-HIV infection?
yes no
6. Are you HIV-positive?
yes no
7. Do you have any of these conditions: heart disease, chronic lung disease, chronic alcoholism?

8. Do you have asplenia (including elective splenectomy and persistent complement component deficiencies)?

9. Do you have chronic liver disease?

10. Do you have any of the following conditions: diabetes, kidney failure, end stage renal disease, or are you on dialysis?
yes no
11. Do you work in a healthcare setting?

12. Do you smoke cigarettes?

The website also has a page: "Report Flu Vaccine Shortages" used to report vaccine out-of-stock situations for flu vaccine, and another, "Track the Flu", to join in participation of tracking flu through weekly surveys that require participation that is anonymous.
